The Diabetes Care Team is a nationally recognized program that meets the American Diabetes Association standards for care and education. Our team is comprised of endocrinologists and certified diabetes educators (registered nurses and registered dietitians). Together they provide individual and group education, support and counseling for adults and children with diabetes and their families.
Our program offers information and support that will increase your understanding and empower you to manage diabetes. Our team offers:
- Individual care and education
- Access to support groups
- Community-wide educational programs (available upon request)
- Community resources
- Individual consultation and classes (available with a referral from your primary physician)
- Diabetes classes and individual consultations are a Medicare benefit

"Diabetes Day to Day" Monthly Classes These classes are for people newly diagnosed with diabetes and for people who want better control. Classes are held at Longmont Clinic, which is located at 1925 West Mountain View Avenue.

Class price of $360 includes the two-part class for participant and one family member or friend. You can return at any time within a year at no additional cost. We will also bill your health insurance, as it is a covered benefit for many people, including those with Medicare. "Learn More, Know More" Series This series of FREE interactive programs helps participants address ways to maintain good health and delay cardiovascular disease and diabetes:
Know Your Resistance Monday, August 30 6:00 p.m. - 7:30 p.m. Learn about Insulin Resistance Syndrome (IRS) and develop a "game plan" for preventing or delaying cardiovascular disease and diabetes.
Know Your Nutrition Monday, September 20 6:00 p.m. - 7:30 p.m. Understand the role of nutrition and basic carbohydrate counting in IRS, diabetes, and weight control.
Know Your Healthy Steps Monday, October 25 6:00 p.m. - 7:30 p.m. Understand the role of exercise in maintaining and improving overall health. Learn how increasing the level of exercise over time can improve blood glucose, blood pressure, cholesterol and weight control.
